Insurance
I do not work with insurance companies. If your insurance company offers out-of-network benefits, I can provide you documents to submit for out-of-network reimbursement. Many PPO plans reimburse 60–80% of their allowed amount once the out-of-network deductible is met.
Please refer to my Out of Network Reimbursement Guide if you are calling your health insurance to ask about your benefits.
Payment
Before first visits, a credit card or bank account is kept on file to hold your appointment. Payment is due by the end of the appointment day.
For follow-up visits, payment is due by the end of the appointment day.
Cancellations & missed appointments
To cancel or reschedule an appointment, please provide at least 48 business hours of notice. If you provide less than this notice, you will be charged the full session fee for a cancelled or missed appointment.
If I can reschedule your appointment in the same calendar week, there will be no cancellation fee.
A credit card or bank account must be kept on file for each patient. This may be used for missed appointments and late cancellations.
